Optimum Care Group is a trusted NDIS Registered Provider proudly supporting participants across Victoria since the early rollout of the NDIS. We are committed to delivering high-quality, person-centred supports that empower people with disability to live independently, achieve their goals, and participate fully in their communities.
We understand that every participant is unique. That is why we take the time to carefully match participants with Support Workers and services that best suit their goals, personality, cultural background, communication style, and support requirements. Our experienced workforce includes male and female support workers, multilingual staff, Auslan-capable workers, and team members with backgrounds in healthcare, Psychology, disability services, and community support.
At Optimum Care Group, we are confident in the quality of our services. Unlike many providers, we offer a complimentary one-hour meet and greet, allowing participants and families to meet their Support worker and experience our service before making a decision.

Our Services
Support coordination & Specialist Support Coordination
Our experienced Support Coordinators and Specialist Support Coordinators assist participants to understand and implement their NDIS plans, connect with quality services, build capacity, navigate complex systems, prepare for plan reviews, and achieve meaningful outcomes. We are experienced in supporting participants with highly complex circumstances, multiple stakeholders, housing challenges, Behavioural Support needs, and crisis situations.
Supported Independent Living (SIL)
Optimum Care Group provides Supported Independent Living (SIL) services for participants who require ongoing support to live independently in their own home or shared accommodation setting. Our SIL services focus on building independence, developing daily living skills, increasing community participation, and improving overall quality of life.
We have extensive experience supporting participants with complex behaviours, psychosocial disabilities, acquired brain injuries, intellectual disabilities, neurological conditions, and physical disabilities, including 24-hour support models and active overnight supports.
High Intensity Supports
We specialise in supporting participants with complex medical and Personal Care requirements. Our trained and experienced staff provide safe and professional support in accordance with NDIS High Intensity Support requirements.
