With over 20 years of experience in the disability and aged care sector, we're a registered NDIS, and DVA provider offering integrated allied health services. We provide a comprehensive one-stop solution for people with disabilities, veterans, and elderly clients:
• Support Coordination – Goal-driven, participant-centred coordination that builds trust and empowers independence. We work collaboratively with participants to understand their needs, plan their support, and navigate the NDIS or DVA systems seamlessly.
• Support Workers & In-Home Care – Personalised in-home support tailored to individual needs, from personal care and community participation to respite and live-in care. Our experienced team specialises in supporting complex needs, acquired injuries, and mental health conditions.
• Allied Health Services – Integrated therapy and wellness support to complement care and maximise independence.
• Community Participation – We help participants engage with their community through group outings, social events, and meaningful activities—from Australian Open tickets to Game Days and monthly community lunches.
• Specialised Support – We excel at supporting complex cases, including mental health disabilities, behavioural challenges, and participants with multiple support needs. Our team collaborates with occupational therapists, behavioural practitioners, and social workers.
• Additional Services – NILS financial assistance, VR relaxation therapy, psychosocial coaching, and free 15-minute consultations to explore how we can help.

We work with 55+ active participants, primarily aged 20–65, many of whom live alone with limited family support. Our clients include:

We operate across Western and Northern Melbourne, including suburbs such as Banyule, Moreland, Yarra, Darebin, Nillumbik, Hume, Whittlesea, Brimbank, Port Phillip, Hobsons Bay, Melton, Wyndham, Maribyrnong, and Moonee Valley, with service boundaries extending to Epping (north), Melton (west), and Werribee (southwest).
