uCare iCare is built on a simple belief: every person deserves to be heard, respected, and supported to live life on their own terms. We work alongside participants, families, and carers as genuine partners, listening first and tailoring supports that reflect individual goals, values, and choices.

 

We are committed to empowering people with disability to achieve meaningful outcomes, whether that is building independence, strengthening daily living skills, engaging with the community, or feeling safe and supported in their own home. Our supports are flexible, responsive, and designed to evolve as needs and aspirations change.

 

Supported Independent Living (SIL, Respite, MTA)

uCare iCare operates five Supported Independent Living accommodations located in the northern suburbs of Melbourne. These homes provide high-quality, person-centred support in a safe and respectful environment, tailored to the individual needs of each participant.

 

Personal Care

Our compassionate, trained, and experienced team supports participants with personal care in their own homes, at times that suit them. All supports are tailored to individual needs, there is no one-size-fits-all approach.

 

Community Access

We provide a monthly activity schedule shared with participants, families, and key stakeholders. Activities are planned collaboratively and based on individual preferences, with flexibility to choose from available options across Melbourne or adjust plans to suit personal interests and goals.

 

Household Tasks and Gardening

Dedicated professional cleaners assist participants to maintain clean and comfortable homes, separate from personal care supports. Our specialised gardening team provides regular services based on participant preferences, ensuring outdoor spaces are well maintained and enjoyable.

 

Support Coordination

Our experienced and passionate Support Coordination team assists participants and families to connect with suitable service providers. This service is delivered independently to avoid conflicts of interest, and uCare iCare does not provide other supports alongside Support Coordination, in line with NDIS Commission requirements.
