Philiacare

We are a specialist disability service provider team based in WA Perth Metro. Our care and support services are geared towards supporting clients through rehabilitation, adopting, and promoting life coping skills, and living a quality and purposeful life.

 

Service Delivery:

Being a team, we guarantee our clients continuity of service, reliability, and flexibility. With our nursing and care delivery experience, we establish contact with the potential client and / or representative, going through a basic overview of the care needs in liaison with the client/ representative. This helps Philiacare gain an understanding of the client’s expectation and to ensure proper planning for personalized care delivery.

We work together with the client to establish, days/ time of care delivery, how the care is to be delivered, who is to deliver the care, potential barriers, and how these are to be addressed, client wishes, etc.

We appreciate that clients may have simple or complex care needs and therefore work with the nominated multidisciplinary teams. We also collaborate with other services available in the community such as mental health specialists, GPs, continence advisors, wound care specialists, behaviour therapists etc. to promote holistic care.

 

Service Supports:

Support workers are allocated to clients following careful consideration of clients’ needs, staff skill mix, client’s preferences, this being done in liaison with the client and / or their representative to ensure a perfect match. Our team is well screened considering the relevant regulatory checks: character / Police clearances, working with children, visa screens, training backgrounds.
