Crescent Care Foundation Pty Ltd is a registered NDIS provider and ACNC-registered charity based in Liverpool, NSW, dedicated to delivering high-quality, person-centred disability support services to participants across South Western Sydney and beyond.
Founded on the principles of compassion, inclusion, and cultural respect, Crescent Care supports individuals living with disability to live independently, engage with their communities, and achieve their personal goals. Our services span daily living assistance, community participation, support coordination, and specialist supports — all tailored to each participant's unique NDIS plan and life circumstances.
What sets Crescent Care apart is our deep commitment to culturally responsive care. We recognise that many participants come from diverse backgrounds and deserve support that honours their identity, language, and values.
As a not-for-profit organisation, every decision we make is guided by the wellbeing of the people we serve — not profit. We work collaboratively with participants, families, carers, and allied health professionals to ensure holistic, dignified, and effective support at every stage of the journey.
