Crescent Care Foundation Pty Ltd is a registered NDIS provider and ACNC-registered charity based in Liverpool, NSW, dedicated to delivering high-quality, person-centred disability support services to participants across South Western Sydney and beyond.
Founded on the principles of compassion, inclusion, and cultural respect, Crescent Care supports individuals living with disability to live independently, engage with their communities, and achieve their personal goals. Our services span daily living assistance, community participation, Support coordination, and specialist supports — all tailored to each participant's unique NDIS plan and life circumstances.
What sets Crescent Care apart is our deep commitment to culturally responsive care. We recognise that many participants come from diverse backgrounds and deserve support that honours their identity, language, and values.
As a not-for-profit organisation, every decision we make is guided by the wellbeing of the people we serve — not profit. We work collaboratively with participants, families, carers, and allied health professionals to ensure holistic, dignified, and effective support at every stage of the journey.

Amazing support highly recommended
We are the parents of a 34-year-old young man with special needs who is currently supported through the NDIS. As we have recently begun transitioning him into more independent living, we engaged A**** from C**** C**** as his S**** C**** through the NDIA. Having never worked with a S**** C**** before, we were unsure what to expect; however, as ageing parents, we recognised the importance of having professional assistance to help coordinate our son’s long-term, ongoing needs and supports as he works towards greater independence.
While the process has at times felt overwhelming, particularly given the complexity of the NDIS, it has been reassuring to have A****’s knowledge and guidance to help us make the most of our son’s plan. A**** has supported us with confidence and professionalism, providing clear communication, genuine understanding of our son’s NDIS goals, followed-up on correspondence, and thoughtful recognition of potential barriers. He has also been proactive in identifying alternative options where needed, giving us confidence that C**** C**** has our son’s best interests at heart and providing us with reassurance as we move forward.
