Our Services includes:

1. Community Nursing Care:

Our community nursing services are designed to maintain your health, manage medical conditions, and promote comfort and wellbeing — all within your home or supported accommodation.

 

Our qualified nurses can assist with:

- Medication Administration & Management – accurate dispensing, monitoring, and education.

- Wound Care & Dressing Changes – acute and chronic wound management for faster healing.

- Continence Management – catheter care, stoma support, and personal dignity preservation.

- Chronic Disease Management – diabetes, heart conditions, respiratory and neurological support.

- PEG Feeding & Enteral Nutrition – safe feeding procedures with training for carers and families.

- Tracheostomy & Ventilator Support – complex care performed by skilled nurses.

- Health Monitoring & Reports – ongoing assessment and communication with allied health professionals.

- Palliative & End-of-Life Care – sensitive, compassionate nursing for comfort and peace.

Our nurses also provide education for participants and families, helping you understand your health needs and make informed decisions about your care.

 

2. Supported Independent Living (SIL) and Short-Term Accommodation (STA) are provided in a modern, spacious, luxury living home, including supports such as HPS and SDA where applicable. Our SIL supports assist participants to manage the responsibilities of daily living, enabling them to live as independently as possible in their own home.

 

3. In-Home Support and Community Participation services offer tailored, person-centred assistance to help participants live comfortably and safely at home while remaining active, engaged, and connected within their community.
